Run Report Hints & Kinks


ALS (Advanced Life Support)

  • Note on run report
    1. Time of intercept with ALS
    2. Each procedure performed and/or medication given that required ALS
    3. Time back en-route to the hospital

General

  • A run report will always be completed if the ambulance leaves the garage due to an incident. It must contain the crew member's names, times, and mileage away from the ambulance bay for a page whether or not a patient is transported or contacted.
    • Destination Determination is always "Closest Facility" unless the Destination is outside Rochester or Chatfield.
    • Primary Role of Unit is always "BLS Ground" which is our license.
  • The run report needs patient information for each person in an incident where the patient agrees to assessment, treatment and/or transport. If the patient then decides against transport the patient needs to sign the Refusal of Treatment/Transport. If the patient refuses to sign the Refusal of Treatment/Transport note that in the narrative.
  • If the ambulance is canceled before arriving on scene note that and the reason for the call being cancelled in the narrative.

Signs & Symptoms

  • Primary Sign / Symptom check box purpose - Only one of these boxes should be checked indicating the patients primary sign / symptom.
  • Secondary Sign / Symptom check box(s) purpose - Check as many secondary signs / symptoms as apply to your patient.
  • The check box "Change in Resp" should be read as "Change in Responsiveness" or "Change in Mental Status".
  • With cell phone use so prevalent now, (even with our younger patients) it is important that when you collect information for a minor that the phone number you get is for their parent or guardian.

Signatures

  • "...we cannot sign on behalf of the patient. Nor can we sign our own name or the patient's name on the signature form where the patient would sign. If the patient is unable to sign and an authorized representative is not available, then simply document that in the narrative. You can write "Patient Unable to Sign" on the signature line, but you must explain why in the narrative."
  • When transporting a minor try to get the names of both parents for the Guardian field. Sometimes the parents have different last names and/or financial responsibilities to the minor.
  • No need to ask a minor to sign anything on the run report. It has no legal bearing. If a parent or guardian is not available note that. The signature of the medical staff at the receiving end validates the transport.

Attachments

  • In a paper report (PCR) in the medication section of a run report, "See attached list" is permissible as long as the list is actually included with the report! Both St. Mary's and Olmsted hospitals have a copier that could be used to copy an extended list provided by the patient. It is preferrable to actually write the medication list directly on to the report. If there is not enough room in the Medications section you may continue the list in the Narrative section.

Multiple Victim Calls

  • Anyone who, due to mechanism of injury or nature of illness, you believe should be evaluated needs to have a Patient Care Report generated.
  • If you need to get a patient to a medical facility and there are additional individuals who should be evaluated or want to refuse care and/or treatment, a page for additional personnel may be implemented to complete the additional Patient Care Reports.
